You agree to subscribe to 12 consecutive months to the Digital Home Plan or DishPVR Plan. The Digital Home Plan includes 1 Model Dish301 satellite TV system, your choice of America’s Top 150, America’s Top 100 or DISH Latino Dos programming package and 12 months of the In-Home Service Plan. The DishPVR Plan includes 1 Model DishPVR 501 satellite TV system, your choice of America’s Top 150, America’s Top 100 or DISH Latino Dos programming. PVR service and 12 months of the In-Home Service Plan. 1, 2 or 3 additional receivers may be added for $5.00 per month and additional Activation Fees apply.

Prices are for Digital Home Plans only; they do not include state and local taxes or any additional programming selections. If your programming is disconnected for any reason (or if you choose to end your Digital Home Plan) before the expiration of 12 consecutive months, you agree to pay a Cancellation Fee of $240 within ten (10) days of the disconnection or end of Plan participation. At the time of disconnection of service or request to end Plan participation, you must return all of the DISH Network equipment within fifteen (15) days or pay an Equipment Replacement Fee based upon the appropriate equipment (Dish301 - $199, DishPVR 501 - $399, DISH 500 LNBF - $50, Quad LNBF - $149, 4:4 switch and LNBFs - $149). We will automatically charge any applicable fees to the credit card number that you initially provided to DISH Network and/or to any other credit card or debit card number that has been used to make payments to DISH Network. At the end of 12 consecutive months, you have the option of returning the hardware with no penalty or continuing on a month-to-month basis at the current monthly Plan rate.

You authorize DISH Network to investigate your creditworthiness. You further authorize DISH Network to report any payment defaults to credit reporting agencies. You acknowledge and agree that DISH Network is not extending credit to you and that the Cancellation of Equipment Fees are not interest, a credit service free or a finance charge. Customers who apply and are approved for Credit Card AutoPay billing will be approved for participation in the Digital Home Plan offers.

DISH Network shall determine eligibility for this offer in its sole and absolute discretion.
